Sometimes we all need a little reminder to be aware of our surroundings. Awareness helps us to anticipate problems and build a system of ongoing checks designed to spot safety problems. A latent failure is an error that hasn’t been manifested yet, but, will be evident when another triggering event occurs that reveals it. They can be caused by loopholes in the system’s defenses, barriers, and safeguards. Sometimes, these failures aren’t discovered until an accident occurs, but with a little vigilance and care, it doesn’t have to happen this way. With vigilance, normal day to day operations will turn up deficiencies in our processes that are opportunities to learn and improve.
